Patchwork (http://patchworkhq.com) is a simple and secure web application that uses social technology to join up professionals across public services. For example, connecting the team around the family leads to earlier interventions and better support for families. By helping professionals communicate better across organisations, Patchwork saves professionals time, supports better multi-agency working, information is shared at the right time in the right way. Patchwork is currently being used to support multi-agency working with children and vulnerable adults – but it has the potential to be used anywhere where it would help frontline staff to have visibility of the other agencies or practitioners working with their clients. This may include areas such as homelessness, domestic violence or probation services.
